The Jacksonville Beach City Council approved a Surf/Fitness Camp Beach Permit at their regular meeting held April 7, 2003. Complete the application form (PDF), Sections 1, 2, and 3 and submit with the following documentation:
- Valid Certificate of Insurance - $1,000,000 naming the City of Jacksonville Beach as an additional insured
- Operator Hold Harmless Agreement
- Photo identification
- Payment of $79.20 payable to the City of Jacksonville Beach
Additionally, each camp operator must have all participants sign a Participant Hold Harmless Agreement and submit copies of same to the Office of the City Clerk.
